Illinois Youth Survey - Parents of Sophomores and Seniors

Attached is a letter announcing the Illinois Youth Survey, which will be given on February 12, 2020. If you, as a parent, don't want your son/daughter to take the IYS Survey, you may print and sign the attached letter and return it to the high school by January 29, 2020. 

If you are o.k. with your son/daughter taking the Illinois Youth Survey, you need not do anything. Signing and returning the attached letter is only necessary if you wish to have your son/daughter opt-out of answering the survey. 

Your son/daughter should share the same letter that you see attached below. They sophomore and senior students were sent a similar message via school email today (1/15/20).

As you will see in that attached letter, the survey is entirely anonymous and is governed by the Institutional Review Board at the University of Illinois, which reviews the survey and the data collection methods to insure that student anonymity is guaranteed and that no individual student results are ever linked back to that student. EAWR and the other high schools in Illinois only receive a general report about overall totals to the survey questions.
